CONNECT Electronic Referral System
The referral system provides an easy to use platform to send, receive and track referrals online. CONNECT is free to use, and training and technical support is provided at no charge.
CONNECT plays a vital role in managing the well being of the whole-person, by allowing service providers, agencies and organizations to coordinate additional services and opportunities for clients, patients and community members.
Who can use the CONNECT system?
CONNECT is appropriate for social service agencies, schools, medical providers, mental health providers, law enforcement, legal services, senior services, faith-based organizations, youth programs and activities, or any business, agency or organization that provides services to families, adults or children in our community.

Teton County Community Health Assessment
Teton County’s 2024 Community Health Assessment (CHA) provides an overview of the current health status in Teton County. This is the third community health assessment that has been done in Teton County; it provides a more comprehensive examination of the many facets that relate to the health and well-being of the community and is a product of collaboration among Teton County Community Alliance for Resiliency and Educational Support (CARES), Teton County Health Department (TCHD) and Benefis Teton Medical Center (BTMC).

Teton County Public Health Strategic Plan
We are pleased to present Teton County Public Health’s first strategic plan for fiscal years 2018 – 2021. This plan is a key tool that guides our work and outlines our goals and objectives as we move into the future. It was developed in conjunction with the Teton County Community Health Needs Assessment and Improvement Plan and aligns with community priorities. This strategic plan represents our firm commitment to work with our communities so that we have healthy places to live, work, learn and play.
