CONNECT Electronic Referral System
The referral system provides an easy to use platform to send, receive and track referrals online. CONNECT is free to use, and training and technical support is provided at no charge.
CONNECT plays a vital role in managing the well being of the whole-person, by allowing service providers, agencies and organizations to coordinate additional services and opportunities for clients, patients and community members.
Who can use the CONNECT system?
CONNECT is appropriate for social service agencies, schools, medical providers, mental health providers, law enforcement, legal services, senior services, faith-based organizations, youth programs and activities, or any business, agency or organization that provides services to families, adults or children in our community.

Teton County Community Health Needs Assessment & Improvement Plan
Our community’s greatest strength is our people and the ways we care for one another. In October 2026, we launched the 2026 Teton County Community Health Needs Assessment & Improvement Plan (CHNA-IP) – a collaborative effort led by Teton County Public Health, Benefis Teton Medical Center, Teton County MSU Extension, and Teton County CARES—shaped directly by the voices and input of local residents and community stakeholders. Guided by the Science of the Positive, this plan focuses on what’s already working well in Teton County—pooling our local resources to strengthen community connections, support resilience across all ages, and make local services easier to find and navigate for everyone.

Teton County Public Health Strategic Plan
We are pleased to present Teton County Public Health’s 2025 Strategic Plan. This plan is a key tool that guides our work and outlines our goals and objectives as we move into the future. It was developed in conjunction with the Teton County Community Health Assessment and Community Health Improvement Plan and aligns with community priorities. This strategic plan represents our firm commitment to work with our communities so that we have healthy places to live, work, learn and play.
